Study of Frequency of Spontaneous bacterial Peritonits in Patents with Alcoholic liver cirrhosis with Ascites

Abstract


Spontaneous bacterial peritonits (SBP) is the development of a monomicrobial infecton of ascitc fuid in the absence of any contguous source of infecton. SBP occurs most commonly in conjuncton with cirrhosis of the liver. This study was conducted to fnd out the frequency of spontaneous bacterial peritonits in patents with alcoholic liver cirrhosis with ascites and to study the clinical and laboratory characterstcs.The study was a cross-sectonal study and was conducted in Shree Birendra Hospital and Bir Hospital, Natonal Academy of Medical Sciences. Total 50 patents with alcoholic liver cirrhosis with ascites presentng in the emergency department or admited as inpatents who provided writen consent were included in the study. Ascitc tap was done in the hospital setup and sent for bacterial culture and sensitvity to various antbiotcs and routne biochemical, cytological and Gram staining. At the same tme blood and urine samples were taken for culture and sensitvity tests. All reports were collected and data analysed.Majority of the patents i.e., 56 percent were between 40-59 years of age. SBP was present in 18 (36%) patents. Out of total 18 cases of SBP, classical SBP was present in 7(38.9%), Bacterascites in 02 (11.1%) and Culture negatve neutrocytc ascites in 9 (50%) patents with SBP. Clinically all patents presented with jaundice and abdominal distension, 92% of patents presented with abdominal pain,60 % with vomitng, 58 % with fever. Such patents are found to have lower ascitc fuid albumin and high PMN cell count. E Coli is the commonest organism isolated from the ascitc fuid cultures followed by streptococcus pneumoniae.Key words: Alcoholic liver cirrhosis, ascites , spontaneous bacterial Peritonits
